JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.

The Care Coordinator serves as a bridge between prospective clients and treatment access. This role promotes HealthRIGHT 360 services, prioritizes client engagement, and works collaboratively to address barriers to care. The Care Coordinator actively partners with staff, Safe Harbor and El Camino Shelters, and community organizations to ensure sustained access to resources and treatment interventions. Acting as a liaison between HealthRIGHT 360, the provider network, and the broader San Mateo County community, the Care Coordinator conducts outreach at Safe Harbor and El Camino Shelters, will be on site to promote services, and connects prospective and enrolled clients to treatment and ongoing support.

This role also provides direct services to clients in both group and individual sessions, either onsite, offsite, or within community and school settings. The Care Coordinator works independently and as part of a team and is expected to meet the weekly productivity targets established for this position.

KEY RESPONSIBILITIES
Outreach, Engagement, and Care Coordination Responsibilities
• Develop and implement outreach strategies to promote treatment services, increase community awareness, and support client engagement.
• Represent HealthRIGHT 360 professionally in the community and at outreach events.
• Connect prospective and enrolled clients to treatment services, community resources, and ongoing recovery supports.
• Coordinate intake scheduling and referral processes with internal staff and community partners.
• Provide targeted case management and assist clients in overcoming barriers to treatment access.
• Develop and maintain collaborative relationships with community agencies, providers, shelters, and other stakeholders.
• Organize and participate in outreach events, educational activities, site visits, and community engagement initiatives.
• Facilitate intakes, groups, and individual counseling services as needed.
• Maintain timely and professional communication with clients, staff, and community partners.
• Work from designated co-located community sites, including Safe Harbor and El Camino Shelters.
Community Responsibilities:
• Facilitate psychoeducational and process groups, clinical days, and other program activities that support client engagement and recovery.
• Provide support to clients in the outpatient community, including crisis intervention, de-escalation, and conflict resolution.
• Participate in team meetings and collaborate with colleagues to ensure high-quality service delivery.
Administration and Compliance Responsibilities:
• Complete accurate and timely documentation, charting, and data entry in accordance with HealthRIGHT 360, funding, and regulatory requirements.
• Maintain compliance with agency policies, procedures, and electronic health record standards.
• Perform additional duties, special projects, and program support activities as assigned.
Productivity and Coverage Responsibilities
• Meet productivity standards and performance expectations established by the organization.
• Coordinate coverage during absences and unplanned events to ensure continuity of services.
• Adjust workload and provide support during periods of increased demand or staffing shortages.
