Essential Job Functions:
- Within 21 days of intake complete the CRAFFT Tool with the resident to assess substance abuse history and to inform treatment planning
- Within 21 days of intake complete the PEARLS Assessment to assess trauma history and to inform treatment planning
- Within 21 days Complete the CANS for all incoming residents.
- Cohesively Collaborate with Social Workers, and Program Managers to communicate about a resident’s behavioral health needs and progress.
- Meet, in-person with residents a minimum of one time per week and their parent/guardians where indicated. Meetings with parents/guardians may often occur in the family home.
- Effectively utilize Collaborative Problem Solving with residents and their families to address their emotional and behavioral health needs.
- Complete CPS Assessments
- Execute Plan B conversations
- Document all CPS activities on APRICOT
- Actively participate in resident’s Large Team Meetings to report on the behavioral health needs and progress.
- Coordinate and make any necessary behavioral health referrals on behalf of the resident and their parent/guardian.
- Remain in close and frequent contact with all external behavioral health providers for all residents of the program.
- While in the programs, arrange for any mobile crisis evaluations as necessary
- Document all interventions on APRICOT through entering progress notes.
- Provide transportation for residents to and from family time, or outside behavioral health appointments when requested.
- Actively participate in staff and clinical meetings.
- Attend and actively engage in all monthly staff meetings
- Attend and actively engage in scheduled Supervision
- Other duties as assigned.
