Occupational Therapist (OT) – Sonoma Post Acute
Location: Sonoma, CA
Pay: Up to $70 per hour
Schedule: Full Time, Part Time, or Per Diem

Responsibilities
Provide occupational therapy services in a skilled nursing setting
Develop and carry out individualized treatment plans
Work with nurses, physicians, and rehab team members to achieve patient goals
Document treatment and progress in accordance with state and federal standards
Educate residents and families on therapy goals and self-care strategies
Qualifications
Current California Occupational Therapist license
Strong clinical and communication skills
Team-oriented with a passion for patient-centered care
Skilled nursing experience preferred, but new graduates are welcome to apply
