Case Manager

Lynx Therapeutics is seeking a dedicated Case Manager to oversee and coordinate comprehensive care for patients, ensuring optimal health outcomes through effective management of services. The Case Manager will be responsible for assessing patient needs, developing care plans, and collaborating with healthcare providers to deliver quality patient support.

Key Responsibilities:

• Conduct initial assessments to determine patients' needs, preferences, and resources.
• Develop and implement individualized care plans in collaboration with patients and healthcare professionals.
• Coordinate services, including medical, social, and community resources, to achieve optimal outcomes for patients.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families about the case management process and available resources.
• Advocate for patients’ needs and ensure continuity of care across various services.
• Maintain comprehensive and up-to-date documentation of patient interactions and care plans.

Requirements

Qualifications:

• Bachelor's degree in Nursing, Social Work, or a related field; Master’s degree preferred.
• Current licensure as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) is required.
• Minimum of 3 years of experience in case management or related healthcare field.
• Knowledge of healthcare regulations, community resources, and effective care coordination methods.
• Strong communication, organizational, and problem-solving skills.
• Ability to work independently as well as part of a multidisciplinary team.
• Experience with electronic health records (EHR) and case management software is preferred.
