What You’ll Do:

• Evaluate and treat post-acute patients with individualized care plans

• Collaborate with other therapy professionals, nursing staff, and families

• Promote functional recovery, independence, and mobility

• Document progress and outcomes according to facility standards

Requirements:

• CA-licensed Physical Therapist (or eligible)

• SNF experience a plus, but new grads welcome!
