Description
The Personal Care Aide works closely with the Director and reports directly to them. This position also works closely with the doctor and nurse to ensure that the quality of life is met.
Lincoln Health is a 15-bed Critical Access Hospital co-located with 22-bed Long Term Care Unit. The facility employs 150-200 employees. Lincoln Park Living Center is 12 bed assisted living. The home employs 6-8 employees, with a Director and an Assistant Director in house Monday-Friday and on weekend emergencies.
ESSENTIAL FUNCTIONS (Personal Care Aide):
1) Provides care as directed by the attending physician to resident requiring assisted living.
2) Reports observations and other pertinent information relate to the care of the resident.
3) Documents objective information related to resident care in the ADL, as well as assisting in the ADL’s with each resident.
4) Provides services that support the care delivered to the resident.
5) Provides input in the formulation and evaluation of the standards of care.
6) Participates in the development of unit objects.
7) Participates in the identification of staff educational needs.
8) Conducts self in a professional manner in compliance with unit and house policies.
9) Participates in the quality assessment and improvement process and activities.
10) Performs other related duties as assigned or requested.

QUALIFICATION REQUIREMENTS:
MINMUM PERFORMANCE STANDARDS:
Performance in the following areas is acceptable when:
GENERAL RESIDENT CARE:
1) Residents are identified using identification photos that are posted in each resident white file, located in the office, before administrating any meds or meals.
2) Resident is approached in a kind, gentle, and friendly manner. Respect for the resident’s dignity and privacy is constantly provided. Confidentiality of all resident information is maintained, as well as all other resident rights, posted by the library.
3) All grievance and complaints made by the resident are reported to the Director.
4) Assistance is provided with admissions/transfers/discharges:
a. Room is stripped completely after resident has left
b. Resident is moved to another room
5) Resident rooms are cleaned and kept up daily
a. Vacuuming
b. Dusting
c. Trashes emptied
d. Beds made
e. Fresh Linens
f. Clean and bleach bathrooms
6) Care plans are reviewed on a monthly basis or when conditions of the resident significantly change.
7) Assistance with resident care is provided as directed:
a. Baths
b. Linens changed as needed or on their specific date
c. Daily Cleaning/Deep Cleaning
d. AM and PM care/peri-care is given
e. Vital signs are taken as directed, blood pressure, temps, pulse ox
f. Assisting residents to and from meals
g. Help and observe changes in a resident’s condition or behavior, documentation and proper notifications are given to Director
h. Record and document information in the ADL and MAR
i. Answer phone calls and welcome visitors.
j. Protect the organizations values by obeying HIPPA and privacy issues.
k. Serves and protects Lincoln Health and Lincoln Park Living Center community by adhering to professional standards, assisted living policies and procedures, at all levels; federal, state, and local.
l. Update job knowledge by participating in educational opportunities, in-services, and Relias trainings as scheduled.
m. Provides individualized attention, which encourages each resident’s ability to maintain or attain the highest physical, mental, and psycho-social well-being.
n. Fully understands all aspects of resident rights, including the right to be free from restraints and free of abuse.
o. Protects the personal belongings of each resident.
p. Promotes a home-like environment for residents and guests.
8) Assistance with ACTIVITIES OF DAILY LIVING:
a. Personal Hygiene

9) Assistance with INSTRUMENTAL ACTIVITIES OF DAILY LIVING:
a. Basic communication skills
b. Transportation
c. Meal preparation
d. Shopping
e. Housework
f. Managing medications
10) Hallways are cleared of equipment and other hazardous items.
11) The house is kept up and in orderly fashion.
12) Dishes and kitchen is always kept up and cleaned.
MAINTENANCE OF SUPPLIES/EQUIPMENT:
1) All equipment is operated in a safe manner.
2) Broken equipment is reported to Director.
