Position Overview:

Responsible for building and operationalizing a comprehensive patient navigation program that spans acute and ambulatory care environments. Is a strategic thinker and operational leader that bridges clinical and administrative functions, enhances patient access to timely care, and reduces fragmentation across the continuum. Through leadership, innovation, and collaboration, drives measurable improvements in care coordination, patient experience, and clinical outcomes. Plays a key part in shaping the organization’s strategy to ensure every patient receives timely, coordinated, and compassionate care throughout their journey.

Job Description:

EDUCATION:

Graduate of an accredited school of nursing

Bachelor’s degree in Nursing (BSN)

CERTIFICATION & LICENSURE:

Current RN License – State of California

BLS – Basic Life Support Healthcare Provider

TYPICAL EXPERIENCE:

8 years of recent relevant experience

SKILLS AND KNOWLEDGE:

Progressive leadership experience in clinical operations, care coordination, or patient access within a complex healthcare system.

Proven experience managing clinical and non-clinical staff and leading cross-functional, patient-centered initiatives.

Deep understanding of hospital and ambulatory operations, clinical workflows, and access challenges across care settings.

Demonstrated success using data and metrics to inform and drive operational improvements.

Pay Range is $103.75 to $166.00 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
