Job Summary:

The CHPM fellowship pathway is a part time 2-year virtual clinical fellowship in conjunction with a 2-year Master of Science in Palliative Care.  The MSPC is administered through the University of Colorado Anschutz graduate education programs and teaches a diversity of palliative care providers—physicians, nurses (including BSN and advanced practice nurses), physician assistants, pharmacists, social workers, spiritual care providers, psychologists, therapists, counselors and ethicists—to be Palliative Care Community Specialists. Graduates provide high-quality palliative consultation and care to patients and families living with serious and life-threatening illness. The MSPC is foundational for the University of Colorado Community Hospice and Palliative Medicine (CHPM) Fellowship for physicians who wish to become board-certified practitioners in palliative care and hospice. Physicians do their academic coursework in the CHPM and MSPC virtually, while the clinical portion of the CHPM is done in fellows’ own geographic location either with or without direct observation of their clinical interactions with patients.  Clinical reasoning and skill are assessed and documented in patient logs that include patient write-ups, journal clubs, asynchronous self-taped videos of communication skills and documentation of all clinical requirements.  

As a clinical mentor you will meet with students on a regular basis throughout their two years to assess clinical reasoning and progression through ACGME HPM milestones and HPM EPAs.  You will collaborate closely with other mentors, program leadership and faculty to ensure fellows are receiving the clinical support that they need to succeed as an HPM physician. The CHPM mentors will be supervised by the lead of the CHPM clinical mentorship program and the APD/director of the CHPM. It is anticipated that the mentor will work 70-110 hours/year.

Key Responsibilities:

• Meet with a cohort of fellows monthly (first year class) and bimonthly (second year class) to perform chart stimulated recall to assess clinical reasoning

• Review HPM milestones and EPAs with fellows and assess their progress in the development of skills

• Review program requirements with fellows and ensure they have a plan to complete all of them

• Attend every other month mentor group meetings

• Attend every other month meetings with the lead of the clinical mentorship program and CHPM director to discuss fellows in your cohort

• Attend every other month sub-ccc meetings

• Mentor students who request extra help or are asked by program leadership to seek more mentorship.

• Review fellows independent learning plans.

• Document each fellow’s formative progress after each monthly meeting, review quarterly med hub evals with CHPM director and twice a year in CCC summary letter
