About the role
Summary of Performance Expectations.
The Medication Reconciliation Pharmacy Technician (MRPT) role assists in the collection of an up-to-date medication list for patients prior to their admission to the hospital by interviewing patients, reviewing their previous medication records and consulting with their outpatient pharmacies. Additionally, the MRPT will review patients admitted in the last twenty four hours, outside of the MRPT hours to further improve medication home medication lists. The MRPT will obtain medication histories in the Emergency Department as well as in the inpatient areas.
Education and Credentials:
Required
High School diploma or equivalent. Registered with NC Board of Pharmacy and certification from the National Pharmacy Technician Certification Board (CPhT).
Preferred
Formal technical or vocational training.
Work Experience:
Required
Three years of experience as a retail pharmacy technician or as a medication reconciliation pharmacy technician.
Preferred Five years of experience as a pharmacy technician with a combination of retail and hospital experience; experience completing home medication lists as a medication reconciliation pharmacy technician

Within six months of employment the MRPT will have obtained the following: 1. ASHP Medication Reconciliation Certificate Given training and on-the-job experience incumbent should be proficient in the basic aspects of the job within three months of employment date.
Preferred Five years of experience as a pharmacy technician with a combination of retail and hospital experience; experience completing home medication lists as a medication reconciliation pharmacy technician
