Patient Advocate & Concierge Nursing Advisor – Indianapolis
Broad Street Advocates & Private Nursing is launching its presence in Indianapolis and seeking an experienced nurse to help establish and lead our model of care locally.
This role is designed for a clinician who wants to move beyond institutional healthcare and apply their experience in a more personalized, relationship-driven approach to care.
Broad Street’s model combines patient advocacy, concierge nursing, and clinical coordination to support individuals and families navigating complex health situations while remaining in their homes. Because we are private-pay Concierge Health Service, we have the freedom to provide the best possible care. We work with families who want proactive oversight, clinical judgment, and trusted guidance - not episodic or insurance-driven care. No coding, just care.
This role will involve working closely with Broad Street’s Advocacy lead in Chicago to help introduce our model to the market, developing local professional relationships, and serving as a trusted advisor to clients and families.
This is an opportunity to help shape a new type of healthcare role - one that blends clinical insight, advocacy, and relationship-building.
The Role
You will work directly with families and their care teams to help navigate complex health situations, coordinate care across providers, and introduce clinical support when appropriate.
At the same time, you will help establish Broad Street’s presence in the Indianapolis market by building relationships with trusted professionals and service providers whose expertise supports the high standard of care we provide to our clients and families.
The role evolves with the individual. The right candidate may contribute to:
• Client advocacy and clinical guidance • Development of individualized care plans • Coordination with physicians and healthcare providers • Collaboration with private nurses supporting clients in the home • Relationship-building within the professional community • Helping introduce and shape Broad Street’s model of care in IndianapolisThis is not a traditional clinical role and not a traditional home care position. It is designed for clinicians who want to apply their experience in a broader, more impactful way

Qualifications
• Registered Nursing License in Indiana
• Strong clinical judgment and communication skills
• Experience working with complex or chronic health conditions such as neurodegenerative disorders, stroke, cancer, or dementia (preferred)
• Comfort collaborating with physicians and multidisciplinary care teams
