RN Hospice for Nursing Homes in the Saratoga/Washington/Montgomery County Regions
The Community Hospice has a wonderful opportunity for a Per diem RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes.

What you will do:
- You will visit patients in the Nursing home setting to respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families.
- Days are flexible within Monday-Friday 8a-4:30p
- Work with interdisciplinary team.
What you will need:
- Valid NYS RN license required.
- Minimum 1 year RN experience required. 1-2 years long term care RN experience preferred.
- Previous Hospice experience preferred.
- Ability to learn/use an electronic medical record
- Clean Driver's License and reliable vehicle
- Strong interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkers

**Special Per Diem Rates based on Monthly Commitment**
Pay Range for non-per diem full-time or part-time Hospice RN employment: $35.00-$47.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
