Job Description

The RN Staff Nurse New Grad, novice to advanced beginner, provide nursing care to acute care patients while collaborating with the nursing care team and multidisciplinary team to advance the patient plan of care.  Nursing practices is guided by the American Nurses Association (ANA) Scope and Standards of Professional Nursing Practice and Code of Ethics. This role eses guidelines, policy, and procedures to guide interventions for patient care while gaining the experience in understanding context and analytical reasoning in the clinical setting.  Willingly collaborates with more experienced members of the multidisciplinary team to plan patient care.

This position adheres to and is responsible for the safety standards of Centegra Health System including the completion of the mandatory organizational safety standards on an annual basis

Identification of population served by this position: Neonate through Geriatric as determined by department scope of service.

 

Responsibilities:

  • Assesses the healthcare needs of patients/families by completing a physical and psychosocial assessment of patient/family condition.  Includes pertinent data in assessment. 
  • Demonstrates knowledge regarding the biological, psychosocial, ethnic and religious needs of the patient population, in addition to the age-specific learning needs, and incorporates those needs into the individualized plan of care
  • Analyzes assessment data to determine potential nursing diagnoses/patient problems. 
  • Collaborates with patients/families, nursing care team, and multidisciplinary team to identify expected outcomes/goals of care for each patient and contributes to care planning to achieve individualized patient goals.  Recognizes patient/family as an integral member of the healthcare team.
  • Implements the identified plan of care by providing evidence-based interventions. Coordinates patient care for assigned patient load seeking assistance in the planning and implementation phase as needed resulting in satisfactory outcomes. 
  • Identifies elements of care that are appropriate to delegate to unlicensed assistive personnel.  Coordinates patient care needs, including discharge planning, with appropriate departments.
  • Evaluation:  Evaluates effectiveness of plan of care by assessing patient/family response to interventions.  Collaborates with nursing care team and multidisciplinary team to alter plan of care as needed based on patient response.

Qualifications

Required

  • Successful completion of required clinical rotations at an accredited college of nursing
  • BSN preferred, ADN minimal requirement
  • BLS (specialty areas may define additional certification requirements)
  • Illinois Registered Professional Nursing License
  • Physical ability to perform functions of the job, including lifting up to 40 pounds and ability to stand/walk for prolonged period of time
  • Analytical skills necessary in order to gather and interpret routine data, maintain patient records, organize workflow and prioritize patient needs
  • Ability to work in a team environment

 

Preferred

  • Some patient care experience as a nursing assistant or similar role
  • BSN, ADN candidates must agree to complete BSN within 5 years of hire date
  • BLS and ACLS (specialty areas may define additional certification requirements).  National certification in are of specialty (ANCC recognized certification)
  • Illinois Registered Professional Nursing License
  • Ability to assess patient level and department level needs to prioritize resources to support patient care
