Description

Boone Memorial Health is seeking a full-time Registered Nurse (RN) to join our Primary Care team at the Family Medical Center in Madison, WV. This outpatient role offers a supportive, team-oriented environment focused on delivering compassionate, patient-centered care to individuals and families across the lifespan.

As part of our primary care team, the RN will assist with clinical assessments, care coordination, medication administration, patient education, and chronic disease management. This position is ideal for nurses who enjoy meaningful patient relationships, continuity of care, and a steady daytime schedule.

Qualifications

Required:

  • Current RN license in the State of West Virginia

  • Basic Life Support (BLS) certification – training provided if not current

  • Strong clinical skills, attention to detail, and communication abilities

  • Ability to work collaboratively within a multidisciplinary team

Preferred:

  • Prior experience in a primary care, family medicine, or outpatient clinic setting

  • Knowledge of chronic disease management and preventive care protocols

  • Experience using electronic medical records (EMR)

Responsibilities

  • Perform nursing assessments and triage for scheduled and walk-in patients

  • Administer medications, injections, and vaccines as ordered

  • Support providers with exams, procedures, and coordination of care

  • Educate patients and caregivers on medications, treatment plans, and preventive health

  • Conduct follow-up calls, referrals, and care coordination

  • Document accurately in the EMR and uphold patient privacy standards

  • Assist in maintaining inventory, stocking supplies, and supporting clinic flow
