Nurse Care Coordinator-Medicine

This role will be responsible for the implementation of clinical needs for persons who are not optimally engaged in HIV care. The Community Nurse Coordinator will provide home and/or community visits to perform a biopsychosocial assessment and develop and implement a treatment care plan. In collaboration with the Care Team, the Community Nurse Coordinator will address short-term immediate medical and psychosocial needs. They will perform various patient care activities, including assisting with patient flow, triage, communication, taking vitals, giving injections, administering prescribed medication, home medication reconciliation, assisting with patient personal hygiene, administering prescribed medications in the home or community setting and other needs as required. The Community Nurse Coordinator will collaborate with health care providers in the acute care setting, outpatient setting and the community to manage care for high-risk patients. The nurse will conduct visits in office, home and/or community and may need to accompany medically complex clients to their appointments. The Community Nurse Coordinator may support clinical services at the THRIVE Program.  The majority of our client population have need for medication reconciliation, close follow-up care, and other nurse-based interventions.  The nurse will be responsible for data collection, monitoring and some grant reporting.

EDUCATION and/or EXPERIENCE

• Bachelor's Degree, or diploma in Nursing from an Accredited institution
• Current Maryland licensure or eligibility to obtain Maryland licensure
• Excellent customer service and communication skills
• Two to three years clinical experience.
