LOFT Community Services, funded through the Behavioural Supports Ontario project, provides Central Access/Coordination for cross-sector BSO services in the geographical boundaries of the Central Local Health Integration Network (LHIN). Since this time, the LHIN has dissolved to become Ontario Health (Central) however, the geographical boundaries served by BSO Central Access remain. BSO Central Access received all referrals for BSO Services, completes an intake assessment, triages all referrals and then routes the referral to the most appropriate service provider. BSO Central Access maintains and supports the waitlist for those individuals requiring community BSO services. In addition, BSO Central Access provides information and navigation support.
Job Summary:
As the Clinical Intake Coordinator for Behavioural Supports Ontario Central Access team in the Central LHIN, you will act as the first point of contact for Behavioural Support Services and will be responsible for receiving all referrals to Behavioural Support Services. Through a clinical review of referrals, you will triage all referrals, complete a comprehensive Intake Assessment, and ensure referrals are directed to the most appropriate service for the clients and their care partners. You will also assist in crisis intervention, risk management, and urgent care planning as required. You will maintain the waitlist according to program policies and provide support to those individuals on the waitlist for service. Additionally, you will liaise with community agencies, other practitioners and key partners to coordinate care for clients. You will contribute to program development and participate in the research activities of the program.
What You Will Do:
- Receive, review, prioritize, and coordinate incoming referrals for Behavioural Support Services.Conduct intake assessments, gather collateral information, obtain consent, and complete clinical documentation.
- Manage client caseloads, waitlists, and referral tracking to ensure timely access to services.
- Provide crisis intervention, risk assessment, urgent care planning, and appropriate referrals to support services.
- Support the development and implementation of individualized care plans that enhance quality of life and address responsive behaviours.
- Coordinate referrals and linkages with healthcare providers, community programs, and other support services.
- Build and maintain respectful, recovery-oriented relationships with clients, families, caregivers, and community partners.
- Maintain accurate records, ensure confidentiality, and comply with organizational policies and procedures.
- Participate in multidisciplinary meetings, case conferences, training, and team initiatives.
- Collaborate with internal and external stakeholders to promote integrated, client-centered care.
- Perform other related duties as assigned by Program Manager and/or designated.
- Completion of diploma or degree in Social Work or a related field.
- Minimum 1 year of experience supporting seniors and older adults with responsive behaviours associated with mental health, addictions, dementia, or neurological conditions.
