Work Schedule:
- Monday through Fridays: 8am to 4:30pm

Duties / Responsibilities:
As a Registered Nurse II, you will provide comprehensive, relationship-based care to adult and geriatric cardiology patients in both virtual and clinic settings. Responsibilities include telephone triage, nurse visits, transitional care management, advance care planning, disease management, patient education, medication administration, diagnostic testing, and cardiology procedure coordination. The role serves as a key liaison between patients and the care team, supports prescription refill management, collaborates with providers and clinical staff to ensure efficient patient flow, responds to urgent patient needs, participates in quality improvement initiatives, and may travel to additional clinic locations as needed.
Education Requirements:
- A graduate of a state approved or National League of Nursing accredited School of Nursing.
- Registered Nurse licensure or eligible for licensure in the state Delaware, Maryland or Pennsylvania & New Jersey depending on location of employment.
- Two years’ experience as a Registered Nurse, primary care experience is preferred.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (AND) will be considered with an agreement of completion of BSN in 3 years
- Basic Life Support (BLS) required
