By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.<p></p><p></p><p>As a <b>Home Health Physical Therapist</b>, you will:</p><ul><li>Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.</li><li>Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.</li><li>Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.</li><li>Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.</li><li>Document patient observations, interventions, and evaluations promptly and thoroughly.</li><li>Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.</li></ul><h1><br/><b>Use your skills to make an impact </b><br/> </h1><p>Required Qualifications:</p><ul><li><p>Current and unrestricted Physical Therapy license</p></li><li><p>Current CPR certification</p></li><li><p>Experience collaborating with a team of healthcare professionals</p></li></ul><p>Preferred Qualifications:</p><ul><li><p>One year of experience as a Physical Therapist</p></li></ul><p></p><p>Pay Range</p><p>• $59.00 - $83.00 pay per visit/unit</p><p>• $93,000 - $128,000 per year base pay</p><p></p><p>Additional Information</p><p>TB Statement:</p><p>This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.</p><p>Driving Statement:</p><p>This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.</p><p></p><p></p><p> </p><p> </p><p></p><p><b>Scheduled Weekly Hours</b></p><p></p>1<p></p><p><b>Pay Range</b></p>The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
