Description
Supervisor: Clinical Director
Job Summary: A Case Manager instructs individuals and families in the community about health education and disease prevention utilizing a team based approach.
Responsibilities:
- Educates the community on various medical outreach programs and the importance of utilizing these programs.
2. Helps to coordinate care with other health care providers, monitors stability and
independent living.
3. Assists patient in setting up a medical appointment and ensuring the patient keeps
the appointment.
4. Helps patient obtain necessary resources.
5. Documents patient cases including assessment and patients consent to obtain
assistance.
6. Prepares reports as required by HRSA and NCQA to follow a PCMH guideline.
7. Assist with creating care plans for identified individuals who meet care
management.
8. Maintain an excel spreadsheet to track care managed patients.
9. Other duties as assigned.
Requirements
Requirements/Qualifications/Skills/Experience:
Prior experience working with an electronic medical record system ideal but not required. Proficiency in Microsoft Word, Excel, and Powerpoint. Bachelor’s degree from a four-year college or university in psychology, social work, healthcare administration or other related field. Must possess the ability to communicate verbally and non-verbally with clients, coworkers and members of management. Excellent professional customer service skills required.
Work Hours
Full-time: 36-40 hours/week (Monday through Friday)
