Job Description

The Case Manager (CM) is a registered nurse who is responsible for implementing a system of patient care delivery that focuses on the achievement of outcomes within effective time frames and with appropriate use of resources. The CM collaborates with the interdisciplinary team to develop and implement the patient care plan and monitors the plan to ensure the quality, quantity, timeliness, and effectiveness of services.

Minimum Requirements

• Bachelor of Science in Nursing.

• Licensed as a registered professional nurse in the State of Missouri.

• Current BCLS certification.

• Minimum of three (3) years experience working on a medical surgical unit.

• Good written and verbal communication skills. Must be able to read and speak the English language.
