The Advanced Practice Registered Nurse (APRN) delivers medical care to a wide variety of patients. The APRN is also examines and treats patients independently and in autonomous collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. May prescribe medications and order diagnostic tests. Advises patients about continuing care.Performs and documents complete physical examinations and comprehensive health histories. Functions independently to perform age-appropriate history and physical for patients. Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities Implements interventions to support the patient to regain or maintain physiologic stability. Assists with the provision of care in accordance with facility, state, and federal regulations. Monitors the effectiveness of interventions. Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients. Collaborates with multidisciplinary team members by making appropriate referrals. Facilitates staff, patient and family decision making by providing educational tools. Adheres to University and unit-level policies and procedures and safeguards University assets.

Outpatient Oncology Clinics and/or Hospital-based Centers (in addition to Core Job Functions) 

• May travel to main campus and satellites to provide care to high priority patients (HPP) that are

       new to the system and or require follow-up care. 

• Participate in staff meetings and other pertinent meetings related.

  • Round with collaborating physician(s) and other team members at main campus, the hospital, and satellites locations.

• Collaborate with physicians, staff, and leaders in the care of HPPs. 

• Provides patient/family education regarding disease processes, treatment options, treatment,

                             outcomes, possible complications, medications, discharge planning, and lifestyle modification. 

• Assure appropriate transfer/handoff of patient/communication/documentation.

• Maintains and collects data for patient and program evaluation and participates in quality

       assessment and improvement activities.  

• Screen patients for clinical trials and refer to the appropriate primary investigator (PI).

• Precept and/or mentor APP fellows or nursing students.

• Demonstrates awareness of community and hospital resources and assists staff, patients, and families to effectively utilize them.

• Provide educational in-services to staff based on relevant topics.

• Triage of patient symptoms or concerns with collaborating physician/primary oncology care team and support process handoff communication to emergency services or oncology care clinic (OCC).

• Assure new and follow-up appointments are coordinated and expedited as appropriate.

• Refer to cancer support services when applicable.

• Support referrals from internal and external stakeholder (including community partners)

• Rotate on-call high priority patient (HPP) telephone after hours, weekends, and holidays.

• Provide coverage remotely and in-person to areas of need.

• Communicates with SCCC leaders at main and satellite locations of HPP that have upcoming medical appointments.

• Follows up with SCCC leaders regularly on referrals being coordinated.

Inpatient Consultations and Follow-up Care 

• Assist collaborating physician(s) with coordination of care to those patients that are  hospitalized with a hematological or oncological issue.  

• Confirm appropriate outpatient coordination of care when patients are being discharge.  

• Consult with collaborating physicians on the initial consults and follow-up care of hospitalized

       patients.
