Job Summary:
Ensures a focus on antimicrobial therapies and Infectious Diseases (ID) activities. Practices in a collaborative work environment through regular interaction with infection control, microbiology, clinical staff and ID physicians. Job Requirements:
Education and Work Experience:
- Graduate of school of pharmacy: Required
- Advanced practice residency in Infectious Diseases (ID) or a minimum of three years' experience as a hospital clinical pharmacist with at least one-year primary experience in Academy of Students of Pharmacy (ASP) service: Preferred
- Doctorate of Pharmacy (PharmD): Required
- One year's technical experience: Preferred
Licenses/Certifications:
- Pharmacist license in the state of practice: Required
- Registered Pharmacist (RPH): Required
Essential Functions:
- Works some limited clinical shifts either for preference, or to meet the emergent need of the department. Conducts daily Antimicrobial Stewardship Program activities that are defined by the Ministry Application Service Providers (ASP) policy. Serves as a core team member of the ASP team. Participates in daily inpatient interdisciplinary rounds and/or ID rounds. Tracks effectiveness measures.
- Reviews and audits antimicrobial therapies for time-outs and de-escalation for optimized antimicrobial selection, dosage, route and duration. Reviews, prepares and dispenses medication. Provides medication consults. Works under minimal supervision, uses independent judgment requiring analysis of variable factors.
- Works collaboratively with pharmacy staff to develop the most cost-effective ASP. Works on problems of diverse scope and complexity. Assists with regulatory compliance. Assists in making recommendations on process improvement or development of new Pharmacy services.
- Manages lower level technicians. Participates in clinical and quality improvement in ASP activities to promote best practice in antimicrobial therapies. Conducts medication use evaluations and root cause analysis of adverse drug events that are related antimicrobial therapies, including C Difficile infection. Performs formulary evaluation for antimicrobials and reviews requests for restricted antimicrobials.
- Mentors and trains staff on technical skills. Provides input into hiring and promotion decisions for staff. Precepts pharmacy students and/or pharmacy residents.
- Performs other job-related duties as assigned.
