Notre Dame Health CarePedi Pals Nurse Case Manager- RNFull Time (40 Hours)Our Pedi Pals team is looking for a Nurse Case Manager to be part of their team.
The Pedi Pals Nurse Case Manager provides nursing care to our Pedi Pals patients consistent with standards of practice, state, and federal regulations while following hospice protocols and procedures in the administration of care. Nurse Case Managers must consistently demonstrate clinical proficiency, appropriate and timely documentation, and the ability to function as a successful member of an interdisciplinary team. The Nurse Case Manager functions as the key person in providing a true interdisciplinary array of services to every hospice family unit.
- Responsible for the planning, coordinating, providing and documenting care for terminally ill patients and their family members from day of admission through day of discharge.
- Utilizes, recommends, obtains and utilizes physicians' orders.
- Develops, updates, and implements a Pedi Pals care plan that meets each patient’s specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as the organization’s policies and procedures.
- Participates in the on-call rotation as assigned by the Clinical Director.
- Establish home health aide plan of care as well as indirectly and directly supervising the plan of care per regulations.
- Managing frequencies of the interdisciplinary team as it pertains to the patients plan of care.
- Maintains ongoing communication with patients, families, caregivers and care providers.
- Document accurate and ongoing assessment of patient status via a variety of mediums of communication (verbal, written, email, computer documents and databases). Document patient care reflecting nursing interventions, patient response to care, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record. Maintain up-to-date charts and records on patient care and regular communication with the patient’s physician regarding changes in the patient’s plan of care.

Requirements
Graduated from an accredited school of nursing.
Maintains a current RN license in the state of Massachusetts.
Experience as an RN commensurate with one of the following:
One-year acute care experience within the last two years.
or
One year home health care experience within the last two years.
or
One year community health or public health experience within the last two years.
One year hospice experience within the last two years.
Valid state driver’s license and reliable automobile with proof of current automobile insurance.
Demonstrated leadership capabilities with the ability to relate well to staff and other professionals.
Seeks hospice certification (CHPN) within 2 years of hire preferred.
Must maintain a valid driver's license and dependable transportation.
