Sentara Health is seeking to hire a qualified individual to join our team as a Supervisor, Home Care Coordinator - PACE.
Position Status: Full Time -Day Shift
Position Location: Sentara PACE - Norfolk, VA
Overview
Provides oversight and management for the home care department for both PACE sites, to include home care scheduler(s), home care LPN(s), and home care coordinators. Position will have a case load of participants who are followed for assessment for program intake and reassessment, general care coordination, hospital admissions, and post-acute follow up. Position shares the 24/7 responsibility to assist the Director of Nursing for the daily operations of the home care department and care coordination. Supervises home care and care coordination staff to ensure patient safety, provides high quality outcomes and an exceptional patient/family experience. Serves as a technical/clinical resource to team members; and performs work assignments as needed to provide appropriate coverage and to mentor/assist staff.All Registered Nurses who do not have their BSN will be required to sign a BSN Agreement committing to successfully obtain their BSN within 5 years of hire.
Education
- Bachelor's Degree (Required)
Certification/Licensure
- RN (Required) - Active in Virginia
- BLS (Required within 90 days)
Experience
- 3 years of nursing experience (Required)
- 1 year of nursing leadership/supervisory experience (Required)
- Previous Home Care experience (Preferred)
