Lutheran Social Services of Southern California began in December 1944 when a group of Lutheran congregations in San Diego came together to discuss how to better assist families in need. They began discussing how to form a Welfare Commission. Their vision expanded when they came together with another group of Lutheran congregations in Los Angeles having the very same conversation. Lutheran Social Services of Southern California was officially incorporated in 1946 as a 501(c)(3) non-profit social service agency. The mission has stayed true to its earliest beginnings, to be a servant to those in need. Today LSSSC serves thousands of individuals and families throughout Southern California with over 40 programs/services at 16 different locations. We are part of the Lutheran Services in America (LSA) network and strive to serve those in need with dignity and respect.

JOB SUMMARY:
Under the leadership and direction of LSSSC CalAIM Clinical Senior Director or designated supervisor, the Enhanced Care Management (ECM) Senior Program Manager is responsible for leading and managing the daily operations of the ECM program services and supervision of personnel. This position ensures the delivery of high-quality, culturally responsive, whole-person, comprehensive, and integrative continuum of care (CoC) services to members with complex healthcare, mental health and social needs. The Sr. Program Manager implements ECM strategic initiatives, engages in program continuous improvement, and establishes internal teams and external stakeholders’ collaborative partnerships. This position collaborates with the Director to establish program budgets and ensures that the program operates within budget. The Senior Program Manager supervises Lead Care Managers (LCM) and supports their professional development and growth through the provision of training and guidance in person-centered, culturally responsive, and trauma informed care.
Essential Duties and Responsibilities include:
· Ensures that delivery of program services is aligned with LSSSC's vision, mission, and values.
· Provides effective leadership of program operations and personnel.
- Manages and oversees the daily operations of the ECM services, including staffing, planning, scheduling, and coordination of member care services; collaboration with CS services and services to provide a integrated spectrum of care and services to members with complex needs/health-related social needs.
· Reviews and adheres to ECM scope of work for each program according to contract guidelines.
· Develops program work plan and standard operating procedures as appropriate.
- Monitors and evaluates the performance of the ECM services, using data-driven approaches to identify areas for improvement and implement necessary changes.
- Supervises, supports, and evaluates the performance of ECM LCMs and Care Coordinators - providing guidance, training, & mentorship to ensure optimal ECM member/patient outcomes & service delivery.
- Serves as the primary contact to ECM members enrolled in the ECM Program
- Advocates for ECM members to help them navigate the healthcare system and social services.
- Oversees & conducts comprehensive intake assessments & care management planning & coordination in collaboration with members to develop a Member-Centered Care Plan.
- Monitors implementation of Care Plan and recommends revisions or updates as necessary to accomplish the member’s goals.
- Provides staff, interns, and volunteers with ongoing supervision, support, and training.
- Manages crisis intervention responses and involves clinical staff as appropriate.
- Coordinates the recruitment, hiring, training, development, and evaluation of staff.
- Provides back-up of staff positions as needed.
- Supports CalAIM Programs/Services leadership through sharing experiences, providing input, developing and participating in training.
