About the role

The Program Director is a credentialed individual, responsible for daily operations of the skilled nursing facility transfer team and associate length of stay initiatives in collaboration with the command center and other health system departments. Responsible for constituent administrative and business operations, clinical integrity and for compliance with all regulatory requirements and institutional policies and procedures. Reports to the VP of Care Transitions. 

  • Oversees operational integration and day to day management of the following areas as they relate to clinical services (including but not limited to): Finance, Legal Affairs, Contracts, Compliance, IT, Real Estate/Facilities, Human Resources/Labor Relations, etc.
  • Develops and implements ongoing performance/process improvement programming. Identifies patterns or trends that require action. Identifies and implements best practices and relevant guidelines in each area. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.
  • Initiates and coordinates administrative and clinical aspects of program development, including new clinics and operations, new organizational structures, space allocation, access improvements, etc.
  • Monitors expenses. Develops and implements strategies to reduce expenses.  Handles budgeting and tracks / develops strategies to address budgetary constraints and incorporates appropriate business planning into program development including resource allocation.
  • Facilitates inter-programmatic collaboration between all Mount Sinai Health System campus programs, divisions, departments, services and units.
  • Reviews and identifies ongoing educational staff development needs.   Oversees the development and implementation of effective educational orientation programs.
  • Facilitates inter-programmatic collaboration between Mount Sinai Hospital campus health programs and those of Mount Sinai St. Luke’s, Mount Sinai Roosevelt, Mount Sinai Beth Israel, and related off-site clinical services.
  • May oversee Electronic Information Systems coordination and management.
  • Develops (independently or in conjunction with other department professionals and leadership) and maintains management information reporting for service areas, including measures of patient volume, activity, demographics, service utilization, productivity and profitability. 
  • Serves as direct liaison with medical, social work, case management, contracing and nursing leadership on operational issues.
  • Promotes and maintains a safe and therapeutic environment, and an interdisciplinary approach in quality patient care delivery.
  • Schedules, assigns and evaluates the work of reporting subordinates. 
  • Other responsibilities as assigned.
  • Master?s or greater in Nursing, Social Work, or Physical Therapy
  • 7 years minimum progressive clinical/management experience (preferably in a specialty health setting), serving a wide range/large volume of patients.
  • Current NYS licensure (Nursing, Social Work, Physical Therapist)       
  • Demonstrated interpersonal and leadership and organizational skills.
  • \Substantial experience and established competency assuring compliance with the range of regulatory requirements pertaining to the operation of health services.
  • Analytic ability to collect and organize information, identifies problems and trends, and formulate and implement corrective plans and programs.
  • Ability to develop, coordinate, prioritize and allocate resources.
  • MS Office Suite (Advanced)
