Pay: Starting salary of $55,000/year

As Case Manager (Employment Focus), you’ll have a dynamic and critical role identifying and coordinating services for Veterans who are unemployed or underemployed. Our Homeless Veterans Reintegration Program and the Incarcerated Veterans Transition Program's goal is to provide employment support services, ongoing case management, and linkages to employment opportunities for persons served. As part of our Veteran’s service team, the Case Manager will make a difference in the lives of our clients by establishing a supportive relationship and connecting them to resources in order to maintain meaningful employment. Activities may include developing resumes and cover letters for the veteran, conducting assessments, making referrals, and coordinating services with VA Medical Centers and other community agencies. The Homeless Veteran Reintegration Program and Incarcerated Veterans Transition Program measures success on rapid retraining and employment- this is an exciting fast paced work environment, and no single case presented to you will be the same!
This role is hybrid working in our office in Plymouth and working in the local service area. Candidates must have the ability to travel throughout Norfolk, Plymouth, Barnstable, and Bristol Counties to meet with Veterans in person.
Some of your critical responsibilities:
- Provide service coordination between integrated care team and community partners, for persons served and their families.
- Assist and teach resume writing skills; mock interviews; career search techniques. Create Individual Employment Plans (IEPs) and help veterans reassess and adapt their plan while enrolled in the program.
- Finding workforce training opportunities for veterans and helping them enroll.
- Connect with other providers, employers, and employment placement agencies, for referrals and linkages.
- Co-enrolling veterans into Mass Hire (DVOP) after assessment
- Develop, maintain, and document a strong network of referral sources designed to generate positive outcomes for persons served.
- Travel to employment sites to support persons served, monitor progress and address any barriers identified.
- Communicate effectively with client’s integrated care team and VOAMASS programs to rapidly address and correct issues with services or supports- especially employment and education.
- Ensure that person served receive services that support continued employment; addressing assessed needs, promote healthy decision-making, securing financial independence and promoting relationships.
- Participate in program and professional supervision, attend required staff meetings and trainings, and contribute to team-based collaborative planning. Follows professional standards and ensures that all services are documented and maintenance of persons served records is completed.
- Communicate the needs of clients within the agency and to external agencies that offer assistance to both veterans and nonveterans
- Finding care for non-VA eligible veterans including Mass Health enrollment
- Conducting National Outcome Measures with veterans requesting care during intake, and at 30, 60 and, 90 day follow up points.
- Other duties as needed and requested.
The Case Manager role is about making a difference in the lives of the people we serve. You’ll bring the following qualifications:
- BA/AA degree preferred; however, a minimum of 2 years related experience in a case management role may be substituted.
- Knowledge of veteran issues, homelessness, and state & Federal resources highly preferred.
- Computer proficiency is preferred in electronic health records, Microsoft Word and Outlook.
