Position Overview:
As a PRN Hospice/Home Health CNA, you will provide hands-on care, comfort, and emotional support to patients and their families in home and residential settings. This per request needed role offers scheduling flexibility based on patient and organizational needs. You will work collaboratively with an interdisciplinary care team to deliver personalized, compassionate care.

Responsibilities:

• Provide compassionate, patient-centered care to hospice and home health patients in their homes or residential facilities
   
• Assist patients with activities of daily living, including bathing, grooming, dressing, and toileting
   
• Observe, document, and report changes in patient condition to the interdisciplinary team
   
• Offer emotional support and companionship to patients and their families
   
• Maintain a clean, safe, and comfortable environment for patients
   
• Collaborate with nurses, physicians, social workers, and other care team members to ensure coordinated, high-quality care

Requirements

Valid certification as a Certified Nursing Assistant (CNA) in Colorado.

Valid non-restricted Drivers License

Compassionate demeanor and a genuine desire to make a difference.

Excellent communication and interpersonal skills.

Ability to work effectively both independently and as part of a team.

Empathy, patience, and a nonjudgmental attitude toward patients and families.

Prior experience in hospice care is preferred but not required.
