Assesses patient needs for Occupational Therapy and related therapeutic services, develops treatment plans and goals in consultation with other treatment team members, and conducts therapy programs designed to restore normal functioning and enhance patients' well-being.
Essential Job Functions and Responsibilities:
Evaluates patient to determine treatment needs.
- Gathers pertinent information from the medical record, 1.e. diagnosis, history, nature of referral prior to examining the patient.
- Appropriately utilizes testing procedures, observations, interview techniques, formal and informal evaluation tools to assess patient status. Identifies patient problem areas in preparation for establishing treatment goals and plan. Completes initial evaluation findings on Occupational Therapy evaluation sheet.
- Establishes realistic objective and measurable goals aimed at patient improvement.
Plans Rx program based on evaluation.
- Formulates therapy Rx plans and goals specific for each patient which are designed to help patient regarding normal physical and/or mental functioning, adjusting to handicaps, attaining maximum independence, restoring occupational and social skills, etc. Demonstrates clear understanding of indications/contradictions for each modality and treatment technique.
- Designs treatment program in collaboration with physicians, allied health team, patient and family.
- Establishes a treatment program which includes activities aimed at efficient progression toward goal attainment. Including but not limited to: teaching new occupation skills, enhancing physical dexterity, providing support, teaches patient to enhance adjustment to and acceptance of disabilities. Documents and individualized and current treatment plans, short and long term goals.
Administers treatment program in accordance with accepted standards of care and current knowledge.
- Administers treatment program upon receiving written physician orders. Schedules patient for treatment session, coordinating treatment times with team members. Carries out all Rx programs and activities taking patient needs, intelligence, physical/mental capabilities and interests into consideration.
- Reviews medical record prior to each treatment session to ascertain current patient status. Utilizes appropriate skills and techniques in the delivery of patient care based on scientific facts and accepted rehabilitation therapy. Adheres to department protocols for specific patient populations (e.g. amputees, cardiac patient, total joint replacement, etc.
- Complies with department and hospital safety practices in maintaining a safe treatment environment. Demonstrates safe and appropriate utilization of support personnel in the implementation of patient care programs.
- Utilizes treatment time effectively allowing for multiple simultaneities treatment programs when necessary and equal distribution of workload.
Monitors results of treatment and alters programs as necessary.
- Observes patient for any adverse reaction to treatment, documenting significant changes in patient status and notifying appropriate personnel. Consistently evaluates outcome of car rendered and patient response to treatment. (+) Periodically re-evaluates patient to ensure attainment of goals. Standards: Inpatients to be re-evaluated every 5-7 days, outpatients every 2 weeks.
- Alters treatment goals and programs as appropriate to meet changing needs of patients. Takes any supportive steps required to sustain patients motivation, cooperation and commitment to purse Rx goals and objectives.
