 JOB SUMMARY:  Individual will collaborate with pharmacy department and clinic site leaders to support the prioritization  and implementation of clinical ambulatory and specialty pharmacy programs focused on evidence-based  practice to improve outcomes and support strategic growth initiatives. They will interface regularly with  providers and clinical staff at assigned practice site(s) to build, develop, maintain, and measure  performance of all client level clinical programs; develop collaborative drug therapy management  programs, clinical protocols, outcomes analysis, program execution and support training of clinical staff  across sites. This role will also ensure that all collaborative drug therapy management programs and  associated development of, and maintenance of inter-and- intradepartmental clinical protocols, are  compliant with state and national requirements. This role will be highly visible and a trusted resource for  clinical recommendations to support optimal patient care. This position description should not be interpreted as all inclusive. Requirements

 Duties and Responsibilities 

• Support advancement of quality pharmacotherapy care by actively assuring the provision of  safe, efficacious and cost-effective drug therapy for all agency patients to meet patients’ needs with focus on primary care disease states in patients also living with HIV or at risk of HIV  acquisition 
• Provides face-to-face and one-on-one phone-based disease management pharmacotherapy  appointment with patients in need of therapy monitoring or optimization; address all barriers to care to include access, medication adherence, education 
• Utilize integrated and routine communication with all health care professionals and patients  concerning therapeutic regimens with an emphasis placed on the monitoring and  documentation of specific pharmacotherapy outcomes 
• Responsible for the review and completion of prior authorization requests for medications  requested under the pharmacy or medical benefit as needed, with support from pharmacy  technicians 
• Provide leadership and mentorship for clinical pharmacy resources executing program  framework across sites, with direct role in designing and developing collaborative drug therapy  management programs (CDTM)  
• Develop, track and report on clinical metrics for all CDTM and clinical pharmacy services and  programs appropriate to each client site and practice; ability to work independently and  autonomously to build and develop programs 
• Participate in key committees responsible for quality and performance improvement activities  related to patient safety, quality and medication use
• Develops client facing and publication quality clinical outcomes studies on specialty pharmacy  program work, leveraging programs across all client sites to produce outcomes data and  represent agency in local and national conferences or meetings
•  Assist in the planning, monitoring, and managing of budget in clinical pharmacy services in  partnership with pharmacy department leadership

•  Maintains AAHIVP certification through completion and passing the standardized exam and  maintaining required continuation education hours
