Flexibility + Work/Life Balance
- Under the direction of a Registered Physical Therapist (PT), provide skilled Physical Therapy services on an intermittent basis to patients in their homes under direction from the Registered Physical Therapist.
- Under the supervision of the physical therapist, assist in evaluation and development of a rehabilitative plan of care and in periodic re-evaluation as required.
- Participate in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted within 72 hours, also prepares periodic summary reports.
- As directed, perform routine strengthening and therapeutic exercises and assists the patient with the use of self-help devices.
- Together with the PT, instruct the patient and family and assists in teaching exercise programs to the patient and family.
- Observe and record activities and findings in the clinical record and reports to the PT and the physician the patient's reaction to treatment and any changes in the patient's condition.
- Consult with PT regarding the outcome of home visits and reports physical findings in written form.
Requirements
- Must be a graduate of an approved school giving a course of not less than two years for Physical Therapy Assistants.
- Must be licensed as a Physical Therapy Assistant by the State of Florida
- Must have at least one year of institutional/home health experience.
- Must have current CPR certification.
- Must have reliable transportation and possess a valid driver’s license, suitable driving history, insurance and be insurable (without conditions) by Company’s insurance carrier.
- As a condition of employment, must successfully complete background check including but not limited to criminal history, national sex registry clearance, OIG clearance and character references Preferred experience in a home health environment.
