Position Overview:
Provides overall strategic direction of the outpatient behavioral health program (BH) Develops, translates and/or prioritizes overall program goals and strategies working closely with the inpatient and outpatient medical directors. Provides direction and leadership to those delivering services in the day to day clinical and business operations. Works closely with other departments to help support the overall business operations of the organization and program goals. Provides clinical LCSW care. Communicates effectively and builds trusting collaborative relationships with all stakeholders to achieve organizational goals and strategic plans. Demonstrates professionalism through personal and professional accountability, ethics and promoting evidence-based practice.
MINIMUM REQUIREMENTS
- Master of Social Work – LCSW required
- Licensed for 5+ years as a LCSW or equivalent education and experience.
- 5+ of social work experience in a behavioral health setting
- A minimum of 3 years of supervision experience in a behavioral health setting.
- A minimum of 5 years of program management experience: program planning, program management, coordination, training and evaluation.
- NJ/PA license
ESSENTIAL FUNCTIONS
• Provides overall strategic direction of BH services, including regular outpatient, Integrated Behavioral Health, Collaborate Care, Partial Hospitalization and intensive outpatient care programs.
• Works closely with leadership in growth and development of BH programs and services.
• Provides direction, orientation and training when needed for all new social work providers and clinical staff.
• Supervision of clinical staff according to accreditation and scope of practice.
• Oversees the navigation of patients at BH, reports on outcomes, patient registries, and target interventions.
• Develops, translates and/or prioritizes overall system goals and strategies.
• Develops systems to navigate the care of patients through the full continuum of care (crisis and coordination of other disciplines).
• Functions as a liaison and acts as the program contact for all disciplines such as emergency department, inpatient behavioral health, hospital inpatient medical floors, specialty practices, cancer center, & primary care.
• Coordinates and directs care management flow to utilize resources (as) effectively to enhance program service delivery.
• Provides consultation and educational services, to peers and staff, about the needs of patients, treatment plans, best practices and the impact on health care and compliance with treatment.
• Develops programs and best clinical practice in support of patients and families; coordinates programmatic growth through an interdisciplinary team to promote and support the needs of individual departments, stakeholders, patients, and families.
• Working closely with other departments to enhance process of care throughout the services provided by BH.
• Assesses and provides treatment plans to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational and other diverse backgrounds.
• Continually reviews regulatory requirements for clinical services offered (as services of BH) for quality assurance including clinical documentation requirements. (to assure appropriate and mandatory services are provided and documented.)
• Communicates utilizing written or oral presentations to address social work, clinical or organization issues appropriate to the target audience.
• Formulates objectives, goals, and specific strategies that support the organizations mission and vision.
• Envisions and develops new care delivery models when necessary. Utilizes research findings for the establishment of standards, practices and care delivery models in service.
• Coordinates staff development and performance improvement activities (to increase self-knowledge, staff, and colleague awareness of biological, psychological, and social issues and functioning.)
• Assess the psychosocial functioning and needs of patients and their family members. Formulates and implements treatment plans. Conducts mental status examinations and formulates DSM
