Responsibilities

Job Summary:

 

The Colorectal Cancer (CRC) Program Coordinator provides comprehensive administrative support to the Rectal Cancer Multidisciplinary team.

• The coordinator ensures that patient care pathways are followed according to standards and guidelines.

a. Oversees the data collection as required by the NAPRC standards.b. Management of accurate and timely information for clinical decision-making.

• He/she will coordinate patients through the continuum of care in collaboration with the multidisciplinary team.a. Contact outside facility pathology departments to obtain outside slides/reports.b. Contact outside facility radiology departments to obtain outside imaging/reports if the images are not already in the PACs system.c. Capture the relevant patient details for discussion.

• He/she will serve as the liaison between disciplines and any supportive/community services.a. Monitors the patients with suspected or confirmed rectal cancer to ensure presentation at the tumor board.b. Coordinate the navigation of patients through diagnostic and treatment pathways.

• He/she will work closely with marketing and outreach departments to educate referring physicians and facilities on available services, prevention, screening/detection, and treatment programs.

• He/she will be involved in coordinating weekly conferences and database tracking analysis for treatment outcome purposes.

• He/she will utilize protocols to refer patients to appropriate specialists and enrollment in clinical trials.

• Educates patients and families on the transition to Survivorship and the program available at the BCC.

• Set expectations for the post-treatment transition and remain available to patients and families for questions after completion of treatment.

• He/she will demonstrate the knowledge, skill, and coordination to provide care and guidance to the cancer patient from screening to survivorship.

 

This role supports the CRC program by serving as the Colorectal Cancer Program Coordinator and has responsibilities as indicated by the National Accreditation Program for Rectal Centers manual and to support the Colorectal Cancer Program Director.Receives and assists in processing outside referrals for a timely, efficient appointment with the multidisciplinary team. Reviews outside records to ensure all appropriate diagnostic tests, scans and pathology reports are obtained for review before the patient evaluation.Facilitates the daily operations of the multidisciplinary team creating a working environment that functions in an organized and time conscious manner. Works closely with the multidisciplinary team in all specialties to coordinate the patient’s treatment plan. Communicates clearly with all members of the team to advocate the patient and family centered care concept.Coordinates and prepares weekly case conferences involving radiology and pathology reviews with second opinion referrals.
