- 1st Shift: Monday-Friday: 7am-3pm (8 hours)-32/hr
- 2nd Shift: Monday-Friday:3pm-11pm (8 hours) - $32.00/hr base pay + $0.75/hr 2nd shift differential
- 3rd Shift: 5 days a week (including every other weekend)-11pm-7am- $32.00/hr base pay + $4/hr premium

Somerfield Health Center at The Heritage at Brentwood is currently seeking full-time LPN Care Leaders for various shifts. Job Responsibilities:
- As an LPN, supervises direct care of senior resident and nursing personnel on assigned shift
- Manages accurate and timely administration of medications, treatments, documentation, and emergency calls.
- Provides direct care as necessary and complete nursing rounds
Qualifications:
- Active, unrestricted LPN license in Tennessee required
- At least 6 months’ experience working in long-term care setting preferred
- Able to provide emotional, physical, and psychological support to senior residents
- Ability to work as part of a team and to handle multiple tasks safely and effectively
- Excellent communication skills, including the ability to communicate sensitive information with empathy
- Ability to work effectively with frequent interruptions and attention to detail
