Hospice/Palliative Care Social Worker

Position Summary:
We are seeking a compassionate and experienced Full Time Hospice/Palliative Care Social Worker to support patients with serious illness in their homes. This role focuses on facilitating advance care planning, coordinating community outreach and education, connecting patients with essential resources, and guiding families through transitions in care, including hospice enrollment when appropriate.

Job Functions:

• Lead sensitive discussions with patients and families to explore values, care preferences, and long-term goals.
• Collaborate with the interdisciplinary team to ensure patient wishes are reflected in the care plan.
• Assist in completing advance directives, LaPOST forms, and other legal documentation.
• Provide clear, compassionate education on disease progression, symptom management, and available care options.
• Support families in understanding the emotional and practical aspects of caregiving.
• Provide grief support to patients, families, and facilities as needed.  
• Develop and distribute culturally appropriate educational materials.
• Collaborate with case managers, nurses, and physicians to ensure continuity of care.
• Participate in Interdisciplinary Team Meetings weekly and report patient concerns and changes.
• Document all interactions, care planning activities, and transitions in the electronic health record according to policies, procedures and team expectations.
• Ensure compliance with ethical standards and regulatory requirements in social work practice.

Education:

• Master’s Degree in Social Work (MSW) from an accredited institution.

Work Experience:

• Previous experience with Hospice preferred, or a thorough understanding of the Hospice concept of care and service.

Special Skills:

• Expertise in interpersonal relationships.
• Ability to work individually as well as on interdisciplinary team.

Other Requirements:

• Must have own transportation and provide proof of liability insurance which meets state requirements.
• Must be able to travel to and from all patient homes without assistance.
• Must be able to perform all aspects of the position unassisted.
• Must have good communication skills and techniques with facilities, patients, families, physicians, and office staff.
