About the role

Description

Reporting to the Director of Weekend Services, the Home Care RN Weekend Manager manages the weekend clinical resource and field staff. Included in the role is direct oversight of the clinical team, to include their growth and development, planning and priority setting, organizing the team’s work, ensuring all care delivered is of the highest quality and executing on the goals of the Agency.  


Essential Functions

  • Directs staff services requiring nursing or rehab skills, in accordance with the plan of treatment signed by the physician. Demonstrates competency in all skills required for the position, including, but not limited to, venipunctures, wound care, IV care, telehealth and electronic discharge site review.
  • Works with the Home Care Directors, Scheduling and Home Care Coordinator to set up the assigned weekend visit schedule and assists with Friday afternoon referrals. 
  • Reviews weekend assignments for client acuity levels and possible client and family problems related to agency care/services.
  • Receives weekend referrals from healthcare providers and initiates admission process. Reviews electronic discharge sites. Receives referral documentation and reviews patients needs. Assigns visit staff as needed.
  • Supports established Agency productivity standards through education and performance management.
  • Reports weekend activities, emergent needs, staffing issues to appropriate program manager as soon as possible.
  • Assists the Clinical Resource Nurses with triage as needed.
  • Orients staff to include teaching and documentation requirements, and expectations around the quality and quantity of services.
  • Coordinates field supervisory visits of weekend staff, in accordance with agency policy and procedure. Utilize appropriate discretion in regards to increasing field supervision when performance issues arise. 
  • In conjunction with the Director of Weekend Services, address performance deficiencies. 
  • Participates in the on-call rotation.
  • Supports responsibilities related to the Director of Weekend Director, when the need arises.  

Other Job Functions

  • Participates in the agency’s quality improvement program.
  • Provides coverage during week days/after-hours/holidays as requested by supervisor.

Expected Hours of Work

The employee will be scheduled to work Friday, Saturday, Sunday & Monday; 8:00 a.m. – 5:00 p.m. (Open to 24-32 hours per week)


Holidays that fall on normally scheduled work days are required unless a time off request is approved in advance. Would be responsible for back-up on call in the event that the Director of Weekend Services is off.

Requirements

  • Associate’s degree in nursing required, Bachelor’s degree preferred.
  • Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)
  • Minimum of one year of nursing experience in Home Care, Medical Surgical or Acute Care.
