A Brief Overview
Lead Community Health Workers (CHWs) works one-on-one with patients or families to set and achieve health goals. Responsibilities include the identification and setting of health goals and follow-up provision of social support, health system navigation, community resource linkage and behavior change counseling. This is an autonomous position which requires a consistent drive for excellence, ability to make appropriate and timely decisions with minimal supervision, and effective problem-solving skills. CHWs are responsible for thorough and accurate documentation and working collaboratively within a multidisciplinary patient care team. In addition to patient care responsibilities, Lead CHWs have additional leadership duties within the organization. Promotes healthy lifestyles, manage group dynamics and serves as a resource. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.
What you will do
- Educates clients on health management and provide support for chronic conditions.
- Facilitates communication between providers and clients, monitor service compliance.
- Leads organizational projects and participate in strategic planning.
- Acts as a mentor and support person for Community Health Workers, specifically by providing guidance, training and advice to new staff and through ongoing CHW professional development activities. Provides guidance for junior CHWs in areas of patient care, professionalism and community advocacy.
- Participates in leadership decisions within the organization, including hiring new staff, and providing input to strategic direction and prioritization for the Center’s work.
- Acts as a spokesperson the program including but not limited to speaking to the media, presenting to community groups to raise the awareness, and facilitating sessions of CHW and Manager training.
- Conducts training on topics such as home visit safety, outreach principles, etc.
- Works with Managers and Director to identify areas for improvement and makes recommendations to support staff development.
- Presents health information to individuals or families or groups to promote healthy lifestyle decisions and chronic disease management.
- Assists clients in their homes, community, or clinic setting. Communicate to clients/patients the purposes of the program and the impact it may have on their wellbeing.
- Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
- Documents activities, service plans, and outcomes achieved by clients in an effective manner.
- Assists clients in accessing health-related services, including but not limited to obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and/or social services.
- Uses motivational interviewing to help patients create action plans for reaching their health goals. Provide a wide variety of support (including emotional support, referral to community-based resources, social services, clinical services, etc.) based on CHW discretion and available resources.
