About the Role:

New York Oncology Hematology (NYOH) is seeking a dedicated Inpatient Advanced Practice Provider (APP) to join our Malignant and Benign Hematology team in Albany, NY. In this hospital-based role, the APP will collaborate closely with hematologists to manage the care of patients with complex blood disorders, providing high-quality, compassionate care throughout the inpatient setting. Under supervision or in collaboration with practice physicians, involved in the care of individuals, families, and patient groups who are dealing with cancer or other related diagnosis. This care is going to be for hospitalized patients. Responsibilities may include duties related to prevention, screening, patient care, management, and research. May serve as direct caregiver, health care consultant, patient educator, administrator, or researcher. Ensures compliance with OSHA and other regulatory standards and requirements. 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Works with the inpatient team to manage complex benign and malignant hematological disease. 
  • Chart review, individual daily rounds, orders and interprets diagnostic imaging, assesses, plans, and implements care for patients.
  • Initiates and documents appropriate interventions; evaluates and records patient/family response and prescribes therapies as indicated.
  • Care coordination with the interdisciplinary team including, but not limited to, hospitalists, pathologists, radiologists, surgeons, social work, physical therapy, clinical nutritionists.
  • Performs procedures after appropriate training.
  • Assesses patient/family needs and provides education.
  • Promotes wellness and recommends community resources to meet patient and family needs.
  • Incorporates the developing knowledge of evidence-based research into clinical practice.
  • Developing knowledge of reimbursement procedures and documentation requirements.
  • Completes patient charge capture and medical record documentation in a timely fashion.
  • Discharge coordination to ensure safe outpatient transition. May provide case management for patients and their families, given limited knowledge of community resources. 
  • May promote/provide staff education
  • Responsibilities may include weekend or after-hour calls.

MINIMUM QUALIFICATIONS:

Nurse Practitioner

  • Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Nurse Practitioner program. This position is entry-level and typically requires a minimum of 0-3 years post MSN experience, preferably in the chosen specialty. RN state licensure and current registration with the State Board of Nursing in practicing state. Current state licensure as a Nurse Practitioner. Current basic life support (BLS)/CPR certification required.

Physician Assistant

  • Successful completion of an accredited Physician Assistant program. Current NCCPA (National Commission on Certification of Physician Assistants) certification. This position is entry-level and typically requires 0-3 years of experience as a practicing Physician Assistant, preferably in the chosen specialty. Current state license as a Physician Assistant in practicing state. Current basic life support (BLS)/CPR certification required.
