Caring Hands Home Health, Inc. is seeking a dedicated and compassionate PRN RN Case Manager to support our clients in High Point, Archdale, and Thomasville, NC.

Key Responsibilities:

• Conduct in-home assessments and develop care plans
  
  
• Educate patients and caregivers on treatment plans and health management
  
  
• Perform supervisory visits every 90 days or 60 days for CAP cases
  
  
• Document patient interactions in a mobile EMR system
  
  
• Travel within High Point, Archdale, Thomasville, and surrounding areas
  
  
• Collaborate with field staff and office team to ensure high-quality care
  
  

Qualifications:

• Active RN license in NC or a compact/multi-state license
  
  
• Graduate of an accredited nursing program
  
  
• Minimum of 2 years of nursing experience (home health preferred)
  
  
• Strong clinical judgment and communication skills
  
  
• Reliable transportation and mobile device for documentation
  
  
• Ability to work independently and travel for client visits
  
  

Location:
This position serves clients in High Point, Archdale, Thomasville, NC and surrounding areas.

Compensation:
Competitive hourly rate based on experience. Weekly pay. This is a PRN (as needed) position.
