About the role

Provides support to and builds relationships with high risk, homeless and mental health patients who are frequent emergency department admissions and /or who discharged from the hospital to ensure a successful transition from ED/ hospital to the community and to reduce re-admissions to the  ED/hospital.  Support includes ensuring patient has housing, follow-up physician and ancillary appointments, arranges transportation to medical appointments if needed, and is receiving the equipment and services prescribed in the patients discharge plan. This will be accomplished by assisting families in accessing community and personal resources that will enable them to have successful living experiences.  Serves as a link between organizations serving the homeless and the Medical Center to improve coordination of services.  Represents hospital on Homeless Task Forces.  Supervises service learning and social work interns.

Required qualifications:

  • Bachelor's Degree in human services, social work, marriage and family therapy or related field.
  • California Clinical Social Worker License upon hire; or
  • California Marriage and Family Therapist License upon hire.
  • 3 years of experience in working with the community related to vulnerable populations, mental health services and homeless services.

Preferred qualifications:

  • Master's Degree in Social work.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
