Private Duty LPN – Full-Time – Greenfield, TN

 

Overview:Join our Adoration Private Duty team and experience the difference of one-on-one in-home care. We’re currently seeking a compassionate and skilled LPN for full-time night shifts in Greenfield, TN. This role is perfect for nurses who value personal connections and want to make a real impact with a consistent patient.

 

Highlights:

• 3/4 split schedule

• 11-hour night shifts: 7:00 am - 6:00 pm

• Full-time hours (84 hours every two weeks)

Patient Needs:

• G-Tube care

• Seizure monitoring and precautions

• Total care needs in a home setting

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family teaching according to plan outlined by the RN
• Monitor patient's vital signs and make specific measurements and observations
• Monitor and record intake and output. Report significant changes immediately
• Observe, assess and record patient's physical and mental condition and reports any changes
• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

• Licensed Practical Nurse / LPN license in good standing
• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
• Active CPR certification
• Familiarity with electronic health records
• Strong problem solving and critical thinking skills
• Reliable transportation, valid driver's license and current automobile insurance
