The Pharmacy Resident PGY1 is responsible for working with multidisciplinary health care teams to achieve optimal outcomes. Compounds and dispenses medications and pharmaceutical supplies upon receiving physician orders. Reviews and evaluates drug regimens, which include the medication profile to identify issues that require pharmacy intervention or monitoring. Provides pharmacy services and drug information to customers as necessary.

GENERAL DUTIES

  • Functions as a pharmacist to perform basic responsibilities in compliance with Board of Pharmacy, Drug Enforcement Agency (DEA), and The Joint Commission (TJC)
  • Prepares, dispenses, and manages medications to support safe and effective drug therapy for patients.
  • Participates in patient care rounds with multi-disciplinary teams to make pharmaceutical interventions and evaluate patient progress.
  • Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes.
  • Collaborates with the healthcare team to provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regimens and multiple medications following a consistent patient care process.
  • Provides general and patient specific medication information to patients, providers, nurses and other health care professionals. Interacts routinely with physicians and nurses to provide optimal pharmaceutical care to patients.
  • Counsels patients in medication attributes and the proper use of medications
  • Participates in collection of data for medication utilization evaluations (MUEs)
  • Demonstrates the ability to manage formulary and medication-use processes, as applicable to the organization
  • Serve on hospital committees and present material to committee members as needed (e.g. Pharmacy & Therapeutics Committee, Antibiotic Stewardship Committee, etc.)
  • Complete a research project by demonstrating the ability to evaluate and investigate practice, review data, assimilate scientific evidence to improve patient care and/or the medication use system.
  • Employ appropriate preceptor roles when engaged in teaching (e.g. pharmacy students, technicians, or other health care professionals).  Serve as a preceptor for pharmacy students completing clinical clerkships.
  • Provides educational in-services for pharmacy and multidisciplinary staff as necessary
  • Accurately defines educational needs with regard to target audience (e.g., individual versus group) and learning level (e.g., health care professional versus patient)
  • Staffing is a requirement for all residents and based on specified weekend schedule provided at the start of the residency program. 
  • Demonstrates understanding of the factors that influence Pharmacy departmental planning and the impact of those factors on the strategic planning process
  • Participates effectively on committees or informal work groups to complete group projects, tasks, or goals
  • Demonstrate personal, interpersonal, and teamwork skills critical for effective leadership
  • Apply a process of ongoing self-evaluation and personal performance improvement. Regularly integrates new learning into subsequent performances of a task until expectations are met.


EXPERIENCE QUALIFICATIONS

  • Preferred: Previous experience in Clinical/Direct Patient Care


EDUCATION QUALIFICATIONS

  • Required: Graduate of an ACPE accredited Doctor of Pharmacy degree program; OR Bachelor of Science in Pharmacy; OR Foreign Pharmacy Graduate Examination Certification (FPGEC)

LICENSES AND CERTIFICATIONS

  • Certification Name: Pharmacist Intern
    • Required prior to start of program.
    • Issuer: Louisiana Board of Pharmacy

  • Certification Name: Pharmacist
    • Required before October 1 of the year starting the residency.
    • Issuer: Louisiana Board of Pharmacy
