Aligned Orthopedic Partners is recruiting an experienced Patient Care Coordinator to join our diverse group of talented professionals and support our Total Joint Program. You will report directly to the Clinical Director of Operations (CDO). 


What you will do:

  • Participate in the day-to-day management of our Total Joint Programs alongside the Total Joint Program Manager.  
  • Prepare all total joint patients for their upcoming surgery 
  • Advanced understanding and utilization of PatientIQ, are patient engagement platform used to deliver information to the patients 
  • Ensure patients are added into EMR at least 60 days in advance of their surgery via PatientIQ. Troubleshoot issues with CDO. 
  • Arrange home PT 30 days in advance of surgery 
  • Enter and document 'patient care management" and “remote therapy monitoring” codes in EHR/APM at least 30 days in advance of patient surgery. 
  • Confirm medication protocols and send in prescriptions with clinical oversight 7-10 days before surgery. 
  • Answer emails/phone calls for patients scheduled for surgery within 24 hours.? 
  • Support questions for post op patients after their surgery with assistance of CDO. 
  • Maintain/manage day to day with purchases including but not limited to: 
  •      Ice machines (rental and cash pay) 
  •      CBD products 
  •      Venodyne compression devices 
  •      Nutrition supplements
