Job Summary:

Work Schedule: 4-10hr. shifts, every other weekend. 9am-8pm
In conjunction with the patient, their family (as appropriate) and the Behavioral Health team, the Behavioral Health Case Clinical Coordinator is responsible for supporting the discharge needs of all patients admitted to the Behavioral Health Inpatient Unit. The Coordinator will assist in the collection, assessment, reporting, and provide recommendations to the BH team on the clinical services and outcomes provided to the BH inpatient ensuring continuous performance improvement.
Responsibilities:
Essential Job Functions
1. Manages the Behavioral Health unit daily activities and outcomes to ensure clinical excellence and safe patient care in the absence of the Behavior Health Director or as delegated to by the BH Director.
- Works closely with the BH Director to develop and revise clinical practices, protocols and policies to align with best practice.
- Serves as the clinical expert in patient care via application of evidence based practice.
- Is a role model for staff in clinical practice guidelines and application.
- Participates in and promotes Nursing Shared Governance program.
- Collaborates with unit employees and leaders to develop care environments that support shared decision making, promotes continuous learning and demonstrates responsibility and accountability.
- Completes follow up on clinical variance reports, patient/family concerns and staff concerns.
- Provides oversight of staffing and ensures unit needs are met as delegated to by the BH Director.
- Gives input to assist with completion of staff performance development/improvement plans.
- Participates and coaches staff in identification, analysis, reduction of and evaluation of risk issues.
2. Service Coordination
- Regularly attends and actively participates in the Behavioral Health treatment team meetings.
- As requested, participates in family meetings, treatment plan update meetings, referral screenings and BOMH meetings to ensure continuity in the case management process.
- Documents specific case management and discharge planning activities in the patient’s record to not only provide record of accountability, but to inform and update the physician and other members of the health care team.
- Develops and updates online learning modules and clinical skills elements
- Identify, plan and implement staff development initiatives to enhance knowledge, validate competency and encourage personal and professional growth
- Facilitate and participate in developing and implementing actions and plans/change initiatives that address unit and organizational learning to improve quality of care
- Develop and lead clinical orientation and progression of new hires to the unit. This includes providing feedback and documentation audit for six month probation period for new employees. Facilitates, evaluates and validates new hire clinical progression and competency.
- Supports unit needs by working as staff/primary care RN with a patient care load as assigned.
3. Discharge Planning
- At the request of the treatment team, coordinates appropriate referrals to community-based resources and services consistent with consumer choice/need and provides service coordination related to discharge planning and transition of care.
4. Business Acumen
- Is vigilant to the day to day business fluctuations and responds promptly to re-prioritized assignments to fit the need
- Performs routine assignments independently
- Seeks direction when indicated for unusual or non-routine duties
- Advises appropriate personnel (i.e., Supervisor) of situations requiring administrative, medical or technical expertise and follow-up
