JOB SUMMARY
Provides home health services consistent with the mission, vision and goals of the Agency.  Occupational Therapy Assistant is a member of the home health team who assists in the provision of occupational therapy under the supervision of an Occupational Therapist to provide treatment of disabilities, diseases, or other conditions of health that is focused on developing, improving, or restoring functional daily living skills, compensating for and preventing dysfunction or minimizing disability. The use of hand therapy and physical agent modalities such as splints, or orthotics and other techniques are provided in accordance with a written plan of treatment prescribed by a physician.  The Occupational Therapy Assistant contributes to the coordination of patient care.

DUTIES AND RESPONSIBILITIES
  • Perform services planned, delegated and supervised by the Occupational Therapist.
  • Contribute to the ongoing interdisciplinary assessment of the patient.
  • Contribute to / assists with evaluation of the plan of care in collaboration with the Occupational Therapist, and in partnership with the patient, representative (if any), and caregivers.
  • Communicate with the supervising Occupational Therapist, all physicians involved in the plan of care and other health care professionals (as appropriate) related to the current plan of care, including patient reaction to treatment and changes in patient condition.
  • Assess ability or disability, relieve pain, prevent disability and/or further deformity, develop or improve body skills, and restore function and maintain maximum performance within the patient's capabilities.
  • Assess and perform procedures that may contribute to the evaluation process by gathering data, administering standardized tests and reporting observations.
  • The Occupational Therapist shall determine the duties the assistant can perform and shall consider the following: clinical complexity of the patient, skill level of the COTA and whether continual reassessment of the patient is needed during treatment.
  • The COTA may contribute to the preparation, implementation, and documentation of the treatment and discharge summary.
  • Instruct patient in use of OT equipment such as reachers, grab bars, bathing, grooming or other ADL aids, braces, splints and other prosthetic and orthotic devices.
  • Participate in teaching / counseling of the patient, family, and caregiver(s).
  • Participate in Agency-sponsored in-service education, utilization review and team conferences.
  • Prepare and submit all clinical case documentation, including clinical and progress notes, and required payroll information according to Agency policy.
  • Document each treatment in the patient’s record and notify the supervising occupational therapist of any change in the patient’s condition not consistent with planned progress or treatment goals.
