AW Health Care is seeking an experienced home care Registered Nurse (RN) for a flexible PRN part-time position.This role offers the freedom to set your own schedule, with an expectation of three to four visits per week, and no weekend commitments. As part of the AW Health Care team, you'll be visiting Medicaid clients in both North and South St. Louis City and County.
Key responsibilities include:
- Scheduling and conducting approximately one hour home visits Monday through Friday for medication setups, initial assessments, six-month assessments, and annual reassessments.
- Communicating with clients’ physicians to ensure comprehensive care.
- Completing minimal case notes.

The registered nurse shall be responsible for patient care, under the direction of patient’s physician and
supervision of the patient care coordinator.
- Makes the initials evaluation visit to the home of the patient.
- Provides nursing services, treatments, evaluation and diagnostic procedures requiring specialized skills of the RN.
- Implements the physician’s plan of a treatment and the appropriate preventive and rehabilitative nursing procedures.
- Observes assesses, educates counsel and provides nursing care for the patient.
- Communicates with other members of the interdisciplinary team in providing for the patient’s total needs.
- Maintains patient’s medical records and nursing observations, actions and patient needs to assure continuity of nursing care. Write complete and accurate notes on each visit.
- Observes signs and symptoms, preparing the clinical and progress notes applicable to the patient.
- Submits progress notes to the patient’s physician. Notifies physician promptly of any changes in the patient’s physical or emotional condition.
- Develops a nursing care plan on each patient.
- Provides support, counseling and education for the family of the patient to assist them in meeting the patient’s health and related needs.
- Supervises, directs and coordinates the services of the Home Care Aides assigned to the patient.
- Evaluates the Home Care Aide’s services. Documents same in patient’s record.
- Attends and participates/ in all agency case conferences and in-service programs.
- Provides in-service education of other Home Health personnel as required.
- Provides community health clinical education for student nurses, medical students, physicians or community representatives as required.
- Participates in Agency quality assurance programs.
- Promotes awareness and utilization of home health services to other professionals, physicians, and the community.
- Other duties as assigned.
Requirements
- Must have valid driver’s license and reliable transportation.
- Experience with Medicaid program is a plus.
- The person filling this position must be able to demonstrate the knowledge and skills necessary to provide the care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status in addition to being able to interpret the appropriate information needed to identify each patient’s requirements relative to their age specific needs and to be able to provide the care needed as described in Aging Well Health Care, LLC.
- Thorough knowledge of principles and methods involved in providing skilled care, safety and infection control in the home should be able to demonstrate techniques and methods and give instruction to patients and family.
- Graduate of an accredited school of nursing. Must have current license from the Missouri State Board of Nursing.
- One year of nursing in an acute hospital setting preferred. Previous home health experience preferred.
- Sound physical and mental health.
- Moderate physical requirements.
