Description

POSITION SUMMARY:

The Community Coordinator will provide specialized administrative and technical support to build and maintain community relationships with partnering organizations.

ESSENTIAL RESPONSIBILITES:

  1. In coordination with the Regional Operations & Project Manager, the Community Coordinator will work to develop a consortium of organizations working together to better meet the needs of the patient population.
  2. Identify organizations in our communities to partner with on implementing a social determinants of health screening tool.
  3. Organize and help facilitate meetings between SHR staff and community partners:  2 group presentations and 4 focus groups to identify, develop, and implement a SDOH screening tool used throughout the network of partnering organizations.
  4. Work with other SHR departments to integrate CAB input into the development of the SDOH screening tool.
  5. Coordinate the development of a referral network that can be integrated into existing EMR systems so that community organizations may respond to the needs of clients identified by the SDOH screening tool.
  6. In coordination with key individuals develop assessment tools to track, analyze and present patient outcome measures.
  7. Evaluate of the effectiveness of the screening tool, and the referral system, to ensure that clients are being connected to appropriate services.

Qualifications

QUALIFICATIONS:

  1. Requires valid Texas Driver’s License, appropriate driving record and auto liability insurance
  2. Current BLS certificate
  3. Experience with EHR (electronic health records) preferred

  • Excellent verbal and written communication skills.
  • Proficient in EHR
  • Excellent organizational skills and attention to detail.
  • Ability to perform well in a fast-paced environment.

  • Sound judgement and the ability to think quickly during emergencies
