We are seeking a Case Manager to join our adult residential program in Nazareth, PA.

 

Position Details

The Case Manager functions as primary liaison to all community hospitals and other social services, mental health centers, intensive case-management units, etc., with regard to any referral for admission and or discharge planning and follow-up contacts for our 10 residents.

 

They will: 

• coordinate the referral process for potential admissions.
• conduct pre-admission screenings.
• ensure all required paperwork is completed for new admission. 
• participate in Recovery team meetings.
• provide any coordination updates. 
• complete all other tasks as assigned.

 

Earn $19.58 per hour.

Qualifications

The ideal candidate will possess the following qualifications:

 

Education and Experience:

• Bachelor's degree in any discipline with a minimum of 4 years' experience working with individuals with mental illness required,  

                  or 

• Bachelor's degree in psychology, Social Work, or a Human Service field with 2 years of experience working with individuals with mental illness preferred.

 

Skills/availability

• Time management, communication, and technology skills are highly valued. 
• Case management or similar experience is an asset! 
• Valid Driver's License and Clean driving record required.  
• Must be able to work Monday-Friday 8am-4 pm.
