Outpatient Substance Abuse Counselor - Evenings

Burkwood Treatment Center is seeking a WI licensed Substance Abuse Counselor to provide treatment and support to assist patients in their recovery from addictions, such as alcoholism and drug addiction.  

**Group facilitation hours are 6:00 PM - 9:00PM 3 days per week**

ESSENTIAL JOB FUNCTIONS:

• Conduct individual and group counseling sessions in an residential treatment setting.
• Responsible for initial assessments, as well as follow-up assessments for patients.
• Prepare individual treatment plan for each patient.
• Maintain patient evaluations, notes, and reports as required
• Assist with modifying problem behaviors and may also provide counseling services to families of patients.
• Provide crisis interventions, as needed.
• Provide case management duties for patient, ensuring individualized quality care.
• Evaluate the needs of the patient and determine if referrals to other programs or facilities is needed.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

• High school diploma or equivalent required.
• Degree in social or health services field from an accredited college or university required.
• License in the State of Wisconsin as a SAC or CSAS required.
