As a Case Manager RN at MercyOne, you will apply advanced psychosocial theory and methods to individuals of all ages, as well as couples, families, and groups, with appropriate regard for the complexity of given problems. Responsibilities include providing advanced evaluative, preventive, therapeutic, restorative, and personal care services to meet the complex psychosocial needs of MercyOne patients and their families, enabling them to better utilize medical care resources and achieve optimal health. This may include providing consultative services to rural hospitals, home care agencies, and long-term care facilities. As assigned, provides social work services to patients at any point on the continuum of care, from consultation to post-discharge follow-up. The role will also involve navigating financial advocacy for patients and maintaining industry knowledge on billable services. Interaction with individuals across diverse age groups may also be required.
ESSENTIAL FUNCTIONS
Communication and Interpersonal Skills: Promote positive relationships with physicians, hospital staff, patients, families, and the public to enhance perceptions of patient care.
Resource Management: Assist in managing resources to achieve quality and financial goals.
Problem Solving: Investigate and resolve barriers to ensure successful outcomes.
Caseload Evaluation: Adjust caseloads based on organizational needs.
Information Sharing: Keep Care Management RNs and social workers informed of institutional updates and departmental communications.
Professional Development: Maintain current clinical expertise and promote growth among colleagues.
Support for Leadership: Assist the Director with personnel evaluations, timekeeping, employee counseling, disciplinary actions, hiring, orientation, and maintain accountability for assigned areas.
Accreditation Support: Help the Director with departmental accreditation requirements.
Comprehensive Assessment: Conduct thorough evaluations of individuals' health statuses, including clinical conditions and support systems, to identify needs and make appropriate referrals.
Interdisciplinary Coordination: Monitor and coordinate care plans that involve the individual and their support services throughout the healthcare continuum.
Documentation: Accurately document case management plans, addressing clinical needs, barriers to care, and resource utilization in a timely manner.
Financial Knowledge: Demonstrate understanding of financial and reimbursement processes to manage medical costs effectively.
Length of Stay Management: Address issues related to length of stay and level of care compliance.
