Lifespark Medical Group Physicians are consultative subspecialists with deep expertise in geriatrics and complex care management. They serve as clinical experts and consulting specialists within an interdisciplinary team that includes Nurse Practitioners (NPs), RNs, LPNs, Social Workers and Pharmacists.
This is a purely clinical role focused on high-touch, value-based care — not high-volume practice. Physicians typically see 4–6 clients per day (in-home or in senior living settings) and spend the rest of their time collaborating with the care team to deliver exceptional outcomes. The Lifespark Medical Group serves highly complex seniors under the umbrella of primary care and urgent response services while emphasizing harm prevention, reduced hospitalizations, and improved quality-of-life outcomes. As a team we do three things for our patients:
Cure: restore them to better health and wellness
Palliate: shift focus to what matters and symptom control when quality and quantity of life conflict
Comfort: transition to meaningful late life experiences by adding an integrated hospice capability to the team at the right time
Position: Physician
RN Salary: $234,000 - $329,000 annually, depending on experience
Service Area: Twin Cities Metro area. Plus, mileage reimbursement of .70 cents per mile.
Schedule: Full - Time / Monday – Friday, 8:00am-5:00pm

What You’ll Do:
- Conduct three primary types of visits:
- New Client Visits – Initial evaluations for new clients as part of a comprehensive team-based geriatric assessment. Training will include utilizing a proven 5-step story telling model. Outputs include personalized acute and chronic care plans that align with prognoses and individual wishes.
- Consultative Visits –as requested by NPs for scenarios such as prominent polypharmacy, challenging advance care planning, misalignment with subspecialty plans, or family/client requesting a physician.
- Same-Day Telehealth Visits – a dedicated portion of FTE is allocated to supporting urgent response consults. Work directly via telehealth with our triage team and on-site mobile urgent responder (MUR). The MUR is a nurse trained to do comprehensive assessments in the home and can provide phlebotomy, intravenous fluids, EKGs and select medications.
- Participate in 1–2 weekly work-remote primary care team meetings to discuss new clients and current clients transitioning to/from the hospital or transitional care units.
- Build strong relationships with clients, families, and team members while promoting evidence-based, person-centered care.
- Collaborate with NPs who serve as the primary care providers for clients under your consultation.
- Blend telehealth and in-person care delivery across senior living communities and client homes.
